• Rendering patient care based on the developed nursing care plan.
• Providing personal care and assisting with hygiene, including necessary baths, oral hygiene and shampoo and changing of bed linens
• Assisting with all activities of daily living such as meal preparations, light, and housekeeping.

• Take and records temperature, pulse, respiration, and blood pressure with appropriate skills and competency.
• Maintain cleanliness of assigned client and work area.
• Functioning in an effective manner in emergencies
• Carrying out basic patient care procedures safely, accurately, and according to agency protocol
• Report all complaints and grievances made by the client
• Report and document all incidents promptly
• Use only authorized abbreviations established by the family/facility when recording information
• Supporting health teaching and patient care delivered by licensed nursing personnel.
• Performing tasks and/or interacting with clients, families, staff, and other personnel in a courteous, honest and compassionate manner.
• Documenting accurate basic information on appropriate forms
• Works effectively with the nursing team to ensure they have adequate information to provide appropriate care.
• Attends and participates in regularly scheduled meetings and service training.
• Pursues continuing educational opportunities.
• Responsible for participation in the quality management plan to assist in identifying and correcting problem areas, and/or improvement of services.
• Running Errands and driving clients to doctor’s appointments
• Notify the RN or the agency office when running late or absent from work.
• Communicate effectively and professionally.
• Performs Related duties as assigned.
• High school diploma or GED preferred.
• CNA and/or GNA license
• Must demonstrate satisfactory completion of any stated mandated training.
• Criminal Background Investigation
• Current First Aid and CPR
• Current PPD /chest Xray
• MMR (immunization record or current titer level)
• Professional and Personal Reference Checks
• Must have reliable transportation and fulfill assignments with reliability and punctuality.
• Must have a valid driver’s license and automobile insurance.
Must satisfactorily complete Buxbells Resources training and orientation program. Must accept responsibility for learning and adhering to Buxbells Resources policies and procedures, be able to function in the home setting with minimal direct supervision, and maintain satisfactory relationships with administrative staff, clients, and family members. Must be able to follow verbal and written instructions and document services provided. Must be genuinely concerned about helping people and have high moral standards of honesty and integrity.
